Yeah. How would you tell if someone is cheating or just really good at aiming?
They don't necessarily but it's easier to manage reputation in a smaller community and different rules can apply so one server might clamp down on cheating others might be more anything goes so everyone gets to play how they want.
It's definitely not infallible but neither is anticheat software. How do you tell if someone is using a DMA cheat? Even kernel level anticheat can't detect that.
