How do you find them reliably? I've even looked up their locations using this map and sometimes the mystery chest just wouldn't be there.

Use https://helltides.com. It shows which mystery chests are up.

I also use Helltides website for mystery chests and mapgenie for the rest.Link to Diablo 4 Mapgenie.
