It works for everyone, regardless of what bits they've got. It's a blood pressure medication that changes bloodflow and just happens to improve blood flow through erectile tissue. If you have a penis that means boners, if you have a clit that means more bloodflow through the clitoris. The effect isn't as obviously dramatic as for penii, but the clit also becomes engorged.

I don't think people outside of the US realize just how prevalent the usage of Viagra and Cialis are here.
