That's because XCOM's accuracy percentage is not exactly a "chance to hit". It's a modifier that determines whether the shot is going to go wildly off-target or not, and by how many degrees.
A 100% accurate shot is always going to use the more accurate cone-of-fire, but it may be as much as 1-2 degrees off, which would matter for longer shots.
And that's why I prefer Phoenix Point's targeting system
