So....turns out you have to tune databases. Who knew?! I set some variables from https://pgtune.leopard.in.ua/ and it looks way better now.
I'm not a DB person but I threw "postgres CPU spiking" into Duckduckgo and got some things to check from stack overflow and server fault.com
