You missed above farron keep. That's near farron keep perimeter.
The last image is also from near farron keep. The poison is an obvious giveaway.
Yeah, I consulted with a friend afterwards and he pointed out that what I said was above Farron swamp was Farron Keep Perimeter, where you find the Farron Coal and the twinblades. (Note: I did not tell him my answers, he got to ask but the last one on his own too.)
For the last one you are it right, tbh. I really have no idea on that one.
Looking back I think I was originally wrong on the last one. I'm beginning to think that's in the dlc.
