Sounds about right for the US.. Low air quality, no Universal health care..
What's the reasons for this?

Probably haven't gotten regulatory approval in that particular country. Some features in smartwatches are considered as medical devices.
My Fitbit for example will stop reading SpO2 and ECG outside of countries that have approved it.
